Liver transplant done at KGMU on 21thAugust 2021 has been successful and patient was discharged from KGMU today. Patient is 44 yearold male, resident of Khadra Lucknow. Patient is employed as a typist in a private firm in Lucknow. He was suffering from advanced stage liver cirrhosis with complains of jaundice and gastrointestinal bleeding. His brother and wife were both ready for donation. His wife proved to be a successful match and donated half part of her liver. Wife (donor) was discharged to home one week after surgery while patient is discharged today, 2 weeks after surgery.

Liver transplant surgery at present is a complex surgical procedure. It involves multidisciplinary management by a team of more than 100 doctors and staff.This case was particularly special as this transplant case required complex reconstruction of the blood supply of liver joining 4 veins instead of normally 2 using special microscopic lens vision and 3d reconstructed technology. All this was done on surgical ice and bath of special solutions while liver was out of body on ice for more than 1 hour. Besides 6 blood vessels, the duct draining bile (bile duct) was also joined to the intestines of the patient. Surgery took about 16 hours to complete, while donor surgery last 12 hrs.All Covid protocols were followed during transplant. Both patient and donor were vaccinated with Covid 19 vaccine 2 months before.

The transplant team was led by KGMU Vice Chancellor LT. GEN. (DR.) BipinPuri PVSM, VSM (RETD.). Surgery team was led by Dr. Abhijit Chandra and Dr. Vivek Gupta of the Department of Gastrosurgery. Dr. ShaleenAgarwal and Dr. SumitGoel from Max institute New Delhi were present during the procedure as per collaboration MoU with KGMU. Many doctors play significant role including Dr. SandeepVerma (Gastrosurgery) , Dr. GP Singh, Dr. Mohd. Parvez, Dr. TanmayTiwari, Dr. RatiPrabha (Anesthesia), Dr. RN Srivastav, Dr. MadhuPathak (Psychologist) Dr. SN Sankhwar (CMS), Dr. SumitRungta (Gatromedicne), Dr. Tulika Chandra (Blood bank), Dr. Amita Jain (Microbiology), Dr. NeeraKohli (Radiology), Dr. AtinSinghai Pathology), Dr. AvinashAgarwal (Critical Care) , Dr. S.K. Dwiwedi (Cardiology) Dr. Suryakant (Pulmonary), Dr Ajay Kumar (Medicine). Mr. Jai Prakash& Mr. SatyaPrakash (Gastrosurgery OT staff), MrNandGopal (Coordinator), other residents and staff.

More than 2 lakh patients suffer from liver cirrhosis in India per year. Most of this cases occur due to alcohol abuse, diabetes, viral infections or immune disorders. Only about 2000 liver transplants are done in India per year. Most of the transplant operations are done in private hospitals with a cost of approximately 30 to 40 lakhs.KGMU being a UP Government institute has brought down the cost to about 8 lakhs. For patients with low income KGMU is utilizing govt. schemes like asadhyarog scheme and UP CM fund to further reduce the price tag to 3 to 5 lakhs. Few patients have also been supported by MrAtul Sharma, President of Awadh International Foundation, Lucknow (A Non-Government Organization).

This is the 12th successful liver transplant of KGMU. With a success rate of more than 90%, KGMU is at par with the success rate of the most advanced centers in the world. KGMU has also been the only institute in UP to perform multi-organ donation, and more than 50 organs have been transplanted in needy patients by collaboration of KGMU with other institutes including AIIMS New Delhi and ARMY R&R hospital New Delhi etc.Organ donation rates in India are about 0.3 per million which is very low as compared to countries like Spain and England with rates of more than 30 per million. If more and more people donate organs after their death, then the need to take organs from near relatives and risk to donors life can be totally avoided. In developed countries more than 99% of organs come from brain dead donors, however an opposite situation in our country where more than 95% of organs come from near relatives. Many patients cannot find matching organ donor in family and for them organs from brain dead is the only option. KGMU has done many social outreach activities and awareness campaigns where more than 1000 donors have registered for their organs. More awareness is the need of hour.
